Orthotic and Prosthetic
services are by appointment.
24 hour emergency on call
services.
Non-emergency appointments are
scheduled for the earliest convenience.
Normal Hours of operation are
Mon. thru Fri. 8 am to 5 pm
Evenings
and weekend appointments are optional.
Patients need a Doctor's
prescription and a copy of a photo I.D. and insurance
card(s) for
treatment.
Most insurance carriers are
accepted, prior authorization is required.
A signature is required for
delivery.
Follow-up appointments are
scheduled as needed.
Complimentary billing services
are provided.
